Dear Valued Patients,

We are writing to inform you that as of March 1, 2026, we will no longer be participating providers with Avesis Vision insurance.

Although we will not be participating with your insurance plan, we are still able to provide you with the same exceptional care and service you have come to expect from our office. These visits would, however, be handled as self-paid visits.

We understand that some patients may prefer to seek care within their insurance network. If you decide to do so, we will gladly provide you with a copy of your patient records to ensure a smooth transition of care.

If you would like to continue seeing us, we are, as always, happy to provide our services, whether it’s for a full comprehensive eye exam or to shop our stylish and fun frames.

We truly value your continued trust in our practice.

To show our appreciation for your continued relationship, we are pleased to offer 20% off all future materials and services, since we will no longer be billing your insurance.
