Office Visits

Our practice is committed to providing you and your family with safe, gentle, high-quality dental care.

We understand that you or your child may feel anxious about visiting the dentist. We are sensitive to your needs, and it is our goal to make you feel comfortable visiting our practice while providing the best care possible.

Your First Visit is All about You

Your comfort, your happiness, and your health.

During your first visit, we will review your dental and medical history forms and take X-rays and pictures of your teeth and mouth. Then Dr. Leonardo Lee will meet with you to discuss your X-rays, help you create your own individual treatment plan, and answer any questions you may have. One of our hygienists will clean and polish your teeth, and inspect your teeth and gums for cavities or early signs of tooth decay or gingivitis.

Bring your Smile

But please also remember to bring these with you to your first appointment:

  • Your completed patient information form

  • Your insurance card(s)

  • Your photo identification

Regular Checkups are an Important Part of Maintaining a Healthy Smile!

The American Dental Association recommends a visit to your dentist every six months for regular checkups and routine teeth cleanings to keep your teeth and gums healthy. Based on your individual treatment plan, your hygienist may recommend a more frequent schedule.

We understand you have a busy schedule, so when you’re preparing to visit our office, please remember your appointments can last between 60 and 90 minutes. Bringing completed paperwork with you will decrease the time spent in our office. Should you need to complete paperwork in our office, please arrive 20 minutes prior to your appointment time.

Office Tour

Designed with our patients' comfort - and the environment - in mind.

When creating our dental and eye care practice, we focused on every detail to ensure a space that promotes both comfort and sustainability. From the layout to the atmosphere, our goal was to make you feel at ease the moment you walk through our doors.

We invite you to schedule an appointment or stop by anytime for a personal tour of our office. You can also explore our space from the comfort of your home through our virtual office tour.

Patient Forms

As a new patient, it is very important to disclose your complete medical history to our doctors.

To save time at your appointment, please take a moment to print and complete our New Patient Information Form before you arrive. Simply bring the filled-out form with you to your first visit.

If you’re unable to open PDF files, you can get Adobe Reader® for free. Still unable to open PDF files? We are more than happy to provide this paperwork to you! We only ask that you arrive 20 minutes prior to your appointment with your insurance card and photo identification.

Please be as detailed as possible when completing your paperwork.
